Volunteer/Employee Risk Management 2008 Instructions:

All volunteers (including but not limited to: coaches, managers, referees, etc.) and administrators affiliated with Pen Bay Soccer Club are required to complete this form. The form (and the entire system) is secure. All of your data is safe and it is not shared with any vendors.

The first change from last year is a feature that allows a volunteer to review the available positions for each club. This is done by clicking the ‘Available Positions’ button. A pop-up window will appear and the user can sort by the club name.

Please note that coaching positions do not become available until the seasonal profiles are completed.

Next select your status for registration. Your choices are:

• I registered in 2007/2008. This is my first registration in 2008/2009.

This is for all returning volunteers who signed up in 07-08.

• I have never registered before. I am new to Soccer Maine.

This is for all new volunteers or those who have never used this system.

• I want to change some of my personal information on file in 2008/2009.

This is for anyone wishing to change contact information in 08/09.

• I want to add another volunteer position.

This is for anyone in the system that needs to add extra positions.

An example of this is a volunteer who coaches multiple teams.

Now click the ‘Online Soccer Maine Risk Management Application’ button.

Complete the form, adding information to any field marked with an asterisk (required). Be sure to include an email address in section III. Mass email will be one of the primary tools of this system and without a valid email address you will miss out on a lot of the Soccer Maine communication attempts. After completing the form, hit the ‘Submit Risk Management Information’ button.
